SELF-ASSESSMENT TOOL

Barrie’s COVID-19 Assessment Centre

If you have mild COVID-19 symptoms, visit Barrie’s COVID-19 Assessment Centre at 490 Huronia Rd, at the Barrie
Community Health Centre, behind the Huronia Urgent Care Clinic.

  • Monday - Sunday, 8 a.m. - 8 p.m.

  • If directed by a physician, symptomatic patients will be swab tested at the centre and provided further

instructions on the next steps.

  • The assessment centre does not provide a return to work notes, nor does the province require them.

  • The assessment centre can see high volumes of patients impacting wait times; dress according to the

weather.

  • Patients are advised to first seek guidance on whether it is appropriate for them to visit an assessment centre

by the online self-assessment tool, calling Telehealth Ontario at 1-866-797-0000 or calling the Simcoe Muskoka

District Health Unit at 705-721-7520.

  • The assessment is provided at no cost to the individual.
